Today, particular attention is paid to the drugs with broad spectrum  activity, which give an ability to use them in patients with so-called undifferentiated epilepsies. In this article, the authors describe their own experience in optimization of antiepileptic therapy in a group of 141 patients with various forms of epilepsy. This new optimized  strategy implies inclusion of modern  broad spectrum  AEDs in treatment  schemes along with the reduction  in use  of narrow spectrum  drugs.  Moreover, polytherapy  was  used  in most  of the cases  after treatment regimen correction. All the measures  allowed to achieve remission  in about half of the patients with epilepsy earlier considered as incurable.</p></abstract><trans-abstract xml:lang="ru"><p>Медикаментозное лечение  эпилепсии требует глубоких  знаний  антиэпилептических препаратов (АЭП): спектра терапевтической  эффективности  и  силы  терапевтического  действия;  побочных  реакций  (особенно   тяжелые и угрожающие жизни побочные  реакции); особенностей фармакокинетики, фармакодинамики, лекарственных взаимодействий; скорости титрования, необходимости в проведении лабораторных исследований на фоне лечения, фармакоэкономических составляющих. Основные свойства обусловлены механизмами  действия антиэпилептических препаратов. В настоящее время особое внимание сосредоточено на применении препаратов с широким  спектром терапевтического действия, что обуславливает возможность назначения  их  у пациентов с так называемыми  недифференцированными эпилепсиями. В статье также рассматривается опыт авторов по оптимизации  терапии эпилепсии у 141 пациентов с различными формами  эпилепсии – включение в лекарственную палитру современных АЭП с широкими  терапевтическими возможностями и сокращение  числа назначений  препаратов с узким  спектром действия. Кроме того, после  коррекции  терапии значительно преобладала  политерапия.
