The study group included 140 children (aged from 2 weeks to 18 months) with neonatal seizures and perinatal brain abnormalities. In these cases, the obstetric and early postnatal history was analyzed; all patients underwent video-EEG monitoring, cervical spine X-ray, neuroimaging, and a Doppler ultrasound test.Results. We found a number of reliable ante- and intra- partum predictors of neonatal seizures. According to the medical documentation from the neonatal pathology departments, local neonatologists have difficulties in diagnosing and verifying the type and duration of seizures. In most patients with neonatal seizures, we detected epileptiform EEG activity, signs of birth defects (according to X-ray) and marked changes (according to neuroimaging) in the cervical spine.Conclusion. The results confirm that neonatal seizures are one of the first symptoms of severe brain damage, including intra-natal damage. Evolution of neonatal seizures into drug-resistant epilepsy and further disability is associated with insufficient knowledge of neonatal seizures, standards for their diagnosis, therapy and multidisciplinary observation.</p></abstract><trans-abstract xml:lang="ru"><p>Цель – проанализировать тактику ведения пациентов с неонатальными судорогами и перинатальной патологией мозга и определить пути профилактики их отдаленных последствий.</p><sec><title>Материалы и методы</title><p>Материалы и методы. Основную группу исследования составили 140 детей в возрасте от 2 недель до 18 месяцев с неонатальными судорогами и перинатальной патологией мозга. В ходе исследования изучался акушерский и ранний постнатальный анамнез, всем пациентам проводились такие методы диагностики как видео-ЭЭГ-мониторинг, рентгенография шейного отдела позвоночника, нейровизуализация и ультразвуковая допплерография.</p></sec><sec><title>Результаты</title><p>Результаты. Доказано наличие достоверных анте- и интранатальных предикторов неонатальных приступов. Анализ выписных эпикризов из родильных домов и отделений патологии новорожденных демонстрирует низкую осведомленность врачей в отношении типов приступов новорожденных и подходов к их терапии. Показана роль видео-ЭЭГ-мониторинга в диагностике приступов. У большинства новорожденных выявлены признаки родовых повреждений на уровне шейного отдела позвоночника по данным рентгенографии и выраженные изменения по данным нейровизуализации.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные в результате исследования данные показывают, что неонатальные судороги являются одним из первых симптомов тяжелого повреждения головного мозга, в том числе интранатального.
