It is the first case of VNS implantation in the Russian Federation. The implantation, follow-up, and programming were carried out in the Bekhterev National Research Medical Centre of Psychiatry and Neurology, Saint-Petersburg.</p><sec><title>Material and method</title><p>Material and method. Female patient, 28 y.o., was diagnosed with DRE more than 11 years ago. The course of her epilepsy deteriorated during pregnancy despite of the appropriate antiepileptic treatment. The EEG, MRI, and laboratory tests did not reveal any focal brain lesion that could necessitate surgical resection. After multidisciplinary consultations, no contraindications to VNS procedure were found.</p></sec><sec><title>Results</title><p>Results. The VNS system was implanted in this patient when she was 18-week-pregnant. The procedure resulted in a seizure reduction and allowed the patient to preserve her pregnancy and deliver a healthy child.</p></sec><sec><title>Conclusions</title><p>Conclusions. VNS Therapy® is safe and effective for the treatment of DRE. This successful case of VNS implantation in pregnancy opens new venues for the treatment of DRE in this group of patients. </p><p>The authors declare the absence of conflict of interest with respect to this publication. All authors contributed equally to this article.</p></sec></abstract><trans-abstract xml:lang="ru"><p>Цель – ознакомить врачей неврологов, эпилептологов, нейрохирургов с первым в Российской Федерации случаем успешной имплантации стимулятора блуждающего нерва (Vagus Nerve Stimulator, VNS) пациентке во время беременности с фармакорезистентной формой эпилепсии (имплантация проведена в отделении нейрохирургии ФГБУ «Национального медицинского исследовательского центра психиатрии и неврологии им. В. М. Бехтерева» МЗ РФ).</p><sec><title>Материалы и методы</title><p>Материалы и методы. Пациентка 28 лет, страдающая на протяжении 11 лет фармакорезистентной эпилепсией с тенденцией к серийному течению. На сроке беременности 12 недель на фоне противоэпилептической терапии развилась серия генерализованных судорожных приступов. Проведенное обследование (ЭЭГ, МРТ головного мозга по эпипротоколу, транскраниальная ультразвуковая допплерография) не выявили очагового поражения головного мозга, которое могло быть субстратом для резекционного хирургического лечения. Лабораторное обследование (анализы крови, мочи, коагулограмма) и консультации специалистов (терапевт, гинеколог, анестезиолог) не выявили абсолютных противопоказаний к наркозу и оперативному вмешательству.</p></sec><sec><title>Результаты</title><p>Результаты. Пациентке проведена имплантация стимулятора блуждающего нерва. В результате проведения данной операции удалось сохранить беременность, купировать эпилептические приступы, устойчивые к воздействию медикаментозной терапии и впоследствии родить здорового ребенка.</p></sec><sec><title>Заключение</title><p>Заключение. VNS-терапия является безопасным и эффективным методом борьбы с фармакорезистентной эпилепсией. Успешные случаи установки стимулятора блуждающего нерва женщинам во время беременности открывают новые возможности лечения фармакорезистентной эпилепсии.</p><p>Авторы заявляют об отсутствии конфликта интересов в отношении данной публикации.
