The study included 31 patients with newly-diagnosed generalized epilepsy with GTCS aged 14–52 years (mean age 25.06±9.3 years), which were divided into two groups depending on seizure severity: Group 1 (n=9, 29%) with &lt;18 points, and Group 2 (n=22, 71%) with ≥18 points. Seizure severity was analysed by using National Hospital Seizure Severity Scale (NHS3). All patients underwent video-electroencephalography monitoring (8–24 hours) with IEA assessment at baseline and at 1, 3, 6 and 12 months after the beginning of treatment. Therapeutic efficacy was assessed using the criteria of seizure absence (medically induced remission), seizure rate decrease by &gt;50% (responders), seizure rate decrease by &lt;50% (insufficient efficacy), seizure rate increase and retention in treatment.Results. Total EAI at baseline was significantly higher in patients from Group 2 (p=0.019). Despite markedly reduced EAI level in both groups, in Group 1 (less than 18 points by NHS3) EAI was significantly lower compared to Group 2 (≥18 points) at all subsequent visits: visit 2 (p=0.038), visit 3 (p=0.035), visit 4 (p=0.047), and visit 5 (p=0.022).Conclusions. Assessing seizure severity may become an additional objective criterion while evaluating treatment efficacy.</p></abstract><trans-abstract xml:lang="ru"><p>Цель: анализ роли показателя тяжести эпилептических приступов и индекса эпилептиформной активности (ИЭА) при впервые выявленной идиопатической генерализованной эпилепсии с генерализованными тонико-клоническими приступами (ГТКП) пробуждения.Материал и методы. В исследование включен 31 пациент с впервые выявленной эпилепсией с ГТКП в возрасте от 14 до 52 лет (средний возраст 25,06±9,3 года), которые в зависимости от степени выраженности тяжести приступов были разделены на две группы: 1-я группа (n=9, 29%) – менее 18 баллов, 2-я группа (n=22, 71%) – 18 и более баллов. Тяжесть приступов анализировали с помощью Национальной госпитальной шкалы тяжести припадков (англ. National Hospital Seizure Severity Scale, NHS3). Всем пациентам проводили видеоэлектроэнцефалографический мониторинг длительностью 8–24 ч с оценкой количественного ИЭА при старте терапии, а также через 1, 3, 6 и 12 мес. Эффективность лечения оценивали по стандартным показателям: медикаментозная ремиссия, 50% и более респондеров (недостаточный эффект – менее 50% респондеров), учащение приступов, удержание на терапии.Результаты. Общий ИЭА на 1-м визите был значимо выше у больных с тяжестью приступов более 18 баллов (p=0,019). Несмотря на существенное снижение ИЭА в обеих группах, у пациентов с тяжестью приступов менее 18 баллов ИЭА во все последующие периоды наблюдения был значимо ниже, чем в группе с тяжестью 18 и более баллов: на 2-м визите (p=0,038), на 3-м визите (p=0,035), на 4-м визите (p=0,047), на 5-м визите (p=0,022).Заключение.
