Every year 200 people per 10,000 subjects around the world suffer from serious brain injuries. Severe TBI can result in long-term disability. Posttraumatic epilepsy (PTE) is one of the most dramatic consequences of TBI with an estimated incidence rate ranging from 2% to 50% based on severity of injury. Conducting studies on PTE poses numerous challenges because epilepsy never develops in many patients with TBI or it may occur more than 10 years after TBI.</p><p>In this review, which includes data from studies conducted by Russian researchers, including us, and foreign colleagues over the last few years (mainly 2017–2022), we analyzed and generalized currently known risk factors, clinical and diagnostic features of PTE in order to increase the awareness about modern methods of laboratory and instrumental diagnostics related to this disease (including electroencephalography and routine/special neuroimaging techniques that allow to identify PTE biomarkers). We also aimed to promote development of preventive strategies for patient management. It has been proved that no clear algorithms for PTE diagnostics and treatment are currently available, which often leads to insufficient patient care.</p></abstract><trans-abstract xml:lang="ru"><p>Согласно статистике на начало 2019 г. черепно-мозговые травмы (ЧМТ) являются наиболее распространенным видом травм и одной из главных причин инвалидизации в России. Ежегодно во всем в мире 200 человек из 10 тыс. получают серьезные травмы головного мозга. Тяжелая ЧМТ может приводить к длительной инвалидности. Посттравматическая эпилепсия (ПТЭ) является одним из наиболее драматичных последствий ЧМТ, частота которого по различным оценкам колеблется от 2% до 50% в зависимости от тяжести травмы. Исследование ПТЭ затруднительно ввиду того, что в ряде случаев может пройти более 10 лет после повреждения, прежде чем начнутся приступы, а у многих пациентов с ЧМТ в анамнезе эпилепсия не развивается вовсе.</p><p>В данном обзоре, включающем результаты наших исследований, а также исследований, проведенных российскими и зарубежными коллегами в течение последних лет (приоритетными являлись научные публикации за период 2017– 2021 гг.), осуществлен анализ и обобщение известных на сегодняшний день факторов риска, клинических и диагностических особенностей ПТЭ с целью повышения осведомленности врачей о современных методах лабораторной и инструментальной диагностики данного заболевания (включая электроэнцефалографию и рутинные/специальные методики нейровизуализации, с помощью которых возможна идентификация биомаркеров ПТЭ), а также данных о потенциальных разработке и внедрении в практику профилактических стратегий ведения пациентов.
