Актуальные классификации АРК отражают основные патологические процессы, лежащие в основе их развития. В обзоре дается современное представление о таких типах АКР, как гетеротопии серого вещества и фокальные корковые дисплазии, которые являются распространенной причиной эпилепсии, нередко устойчивой к медикаментозной терапии. Стремительный прогресс в совершенствовании методов нейровизуализации и молекулярной генетике в последние годы существенно увеличил количество распознаваемых форм АРК.</p></abstract><trans-abstract xml:lang="en"><p>Cortical developmental malformations (CDM) include a large group of heterogeneous brain formation disorders occurring in prenatal period. Current classifications comprise the underlying processes in CDM genesis. The article provides recent conceptions about CDM types such as gray-matter heterotopia and focal cortical dysplasia, which commonly cause epilepsy, often resistant to drug therapy. Rapid advances in improving neuroimaging techniques and molecular genetics in recent years have substantially increased the number of recognized CDM forms.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>Аномалии развития коры головного мозга</kwd><kwd>гетеротопии серого вещества</kwd><kwd>фокальная корковая дисплазия</kwd><kwd>эпилепсия</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Cortical developmental malformations</kwd><kwd>gray-matter heterotopia</kwd><kwd>focal cortical dysplasia</kwd><kwd>epilepsy</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Работа выполнена в рамках государственного задания ФГБНУ «Научный центр неврологии»</funding-statement><funding-statement xml:lang="en">The review was performed as a part of the public assignment of the Research Center of Neurology</funding-statement></funding-group></article-meta></front><body><sec><title>ВВЕДЕНИЕ / INTRODUCTION</title><p>Аномалии развития коры (АРК) головного мозга включают большую гетерогенную группу расстройств, связанных с формированием головного мозга и развивающихся в пренатальном периоде [<xref ref-type="bibr" rid="cit1">1</xref>]. К АРК относят дезорганизацию (нарушение структуры) коры, патологическую миграцию нейроцитов, изменения объема мозга (микроэнцефалия или мегалэнцефалия) [<xref ref-type="bibr" rid="cit2">2</xref>]. Нарушение пролиферации/дифференцировки коры мозга (измененный кортикогенез) может быть генетически детерминированным или возникать в результате экзогенного воздействия.</p><p>В последние десятилетия достигнуты значительные успехи в понимании генетических и морфологических процессов, лежащих в основе АРК [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Среди экзогенных причин выделяют инфекционные, токсические, метаболические и сосудистые факторы [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit2">2</xref>]. Тем не менее во многих случаях причина АРК остается невыясненной.</p><p>Данные о частоте встречаемости АРК варьируются в зависимости от возрастной группы и географической локализации исследования. По оценкам в глобальном масштабе, уровень распространенности патологии составляет примерно 1 случай на 2,5 тыс. населения [<xref ref-type="bibr" rid="cit4">4</xref>].</p><p>Клинические проявления АРК в виде неврологических, когнитивных, поведенческих, психических нарушений чаще возникают в детском и раннем взрослом возрасте [<xref ref-type="bibr" rid="cit2">2</xref>]. Их выраженность зависит от типа корковых мальформаций и вовлечения проводящих путей. АРК могут быть случайной находкой при магнитно-резонансной томографии (МРТ) головного мозга и причиной развития эпилепсии, устойчивой к медикаментозной терапии [5–7]. Диагностика АРК существенно упрощается при использовании клинического, нейровизуализационного с высоким разрешением и генетического исследований [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Как отмечается в литературе, при взятии материала для гистологического анализа в случаях хирургического лечения эпилептогенных АРК необходимо ориентироваться на клиническую картину заболевания, данные электрофизиологических и нейровизуализационных исследований [<xref ref-type="bibr" rid="cit1">1</xref>].</p><p>К общим микроскопическим характеристикам АКР относятся1: ламинарная и столбчатая дезорганизация коры, нечеткость границы серого и белого вещества, нарушение миелинизации проводящих путей, глиоз, гетеротопия нейроцитов, волокна Розенталя, баллонные клетки, гигантские нейроциты, незрелые нейроциты, дисморфоз.</p></sec><sec><title>КЛАССИФИКАЦИИ / CLASSIFICATIONS</title><p>Современные классификации АРК отражают основные патологические процессы, приводящие к их возникновению.</p><p>Открытие генов, белков и сигнальных путей, участвующих в развитии коры мозга, углубило понимание патогенеза АРК [<xref ref-type="bibr" rid="cit1">1</xref>]. Показано, что мутации в одном гене могут ассоциироваться с различными типами АРК, а изменения в разных генах, контролирующих один путь, – вызывать один тип АРК [<xref ref-type="bibr" rid="cit8">8</xref>][<xref ref-type="bibr" rid="cit9">9</xref>]. Но, несмотря на полученные данные, некоторые вопросы, касающиеся определений и классификации АРК, остаются неуточненными.</p><p>Первая классификация, основанная на нейровизуализации и кортикогенезе, была предложена A.J. Barkovich et al. в 1996 г. [<xref ref-type="bibr" rid="cit10">10</xref>] и затем неоднократно обновлялась в последующие годы [<xref ref-type="bibr" rid="cit11">11</xref>][<xref ref-type="bibr" rid="cit12">12</xref>]. Последняя версия 2012 г. [<xref ref-type="bibr" rid="cit2">2</xref>] поддерживает концепцию о нарушениях, возникаю- щих на разных стадиях развития коры мозга (табл. 1). АРК подразделяются на три группы: мальформации в связи с патологической нейроцитарной и глиальной пролиферацией и/или апоптозом, мальформации в связи с патологической миграцией и мальформации в связи с патологической организацией коры головного мозга. Получил признание тот факт, что различные стадии кортикогенеза взаимосвязаны и накладываются одна на другую [<xref ref-type="bibr" rid="cit2">2</xref>]. Кроме того, следует учитывать, что процессы, нарушающие формирование коры головного мозга, могут происходить как в разное время и различных областях головного мозга, так и одновременно [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit9">9</xref>].</p><table-wrap id="table-1"><caption><p>Таблица 1. Классификация A.J. Во время стадии пролиферации и апоптоза пирамидные нейроциты и глиальные предшественники образуются из нейроэпителиальных клеток в областях, прилегающих к боковым желудочкам мозга. Базальные и апикальные отростки глиоцитов функционируют как радиальный каркас для миграции нейронов от эпендимы боковых желудочков до мягкой мозговой оболочки [<xref ref-type="bibr" rid="cit13">13</xref>].</p><p>В случае пониженной пролиферации или усиленного апоптоза нейроцитов развивается микроцефалия, в случае повышенной пролиферации или замедленного апоптоза – мегалэнцефалия [<xref ref-type="bibr" rid="cit1">1</xref>]. При дисгенезии возникают дисморфические нейроциты, характерные для фокальной корковой дисплазии (ФКД) II типа и мегалэнцефалии. Показано, что эти состояния являются следствием мутаций в генах белков метаболического пути mTOR [<xref ref-type="bibr" rid="cit1">1</xref>].</p><p>Миграция нейроцитов начинается на 5–6-й неделях внутриутробного периода, достигает пика между 3-м и 5-м месяцами и завершается на 30–35-й неделях беременности (миграция нейроцитов в мозжечке продолжается до середины второго года жизни) [<xref ref-type="bibr" rid="cit13">13</xref>]. Миграция нейроцитов к поверхности мозга осуществляется несколькими способами. Наиболее распространена радиальная миграция. Несколько генов путем локальной выработки факторов роста регулируют транспорт созревающих нейроцитов в кору мозга [<xref ref-type="bibr" rid="cit14">14</xref>]. Вторым способом миграции является тангенциальный [<xref ref-type="bibr" rid="cit15">15</xref>]. Нарушение миграции нейронов может быть в виде слишком раннего или слишком позднего ее прекращения. Нарушение миграции нейроцитов возникает при дефектах в эпендиме боковых желудочков мозга [<xref ref-type="bibr" rid="cit16">16</xref>] и приводит к перивентрикулярной узловой гетеротопии. Прерывание миграции нейроцитов обусловлено нарушением функционирования сигнального молекулярного аппарата (тубулины, белки микротрубочек, актины), необходимого для перемещения нейронов из субэпендимальной области в кору мозга [<xref ref-type="bibr" rid="cit17">17</xref>]. Мутации в генах, контролирующих эти процессы, приводят к АРК, включая лиссэнцефалию, микроцефалию с лиссэнцефалией и дисгирию [<xref ref-type="bibr" rid="cit17">17</xref>]. Напротив, чрезмерная миграция нейроцитов в лептоменинкс связана с дефектами пиальной поверхности мозга и приводит к полимикрогирии и мальформации по типу булыжной мостовой [<xref ref-type="bibr" rid="cit18">18</xref>].</p><p>Конечный этап формирования коры, называемый постмиграционным развитием, с участием пирамидных и промежуточных нейроцитов характеризуется аксоно-, дендрито- и синаптогенезом [<xref ref-type="bibr" rid="cit19">19</xref>][<xref ref-type="bibr" rid="cit20">20</xref>]. Эти процессы тесно связаны с образованием извилин и борозд мозга [<xref ref-type="bibr" rid="cit19">19</xref>]. Большинство постмиграционных процессов выходит за рамки внутриутробного периода развития плода [<xref ref-type="bibr" rid="cit19">19</xref>]. К изолированным нарушениям постмиграционного развития относят вторичную микроцефалию, ФКД I типа и некоторые формы дисгирии [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>При АРК могут вовлекаться ограниченная область коры мозга (ФКД), несколько долей мозга (квадрантная дисплазия), полушарие мозга (гемимегалэнцефалия).</p><p>В обзоре рассмотрены различные формы АКР, при которых пациенты с легкими или умеренными неврологическими и когнитивными нарушениями обращаются к врачу по причине возникновения эпилептических приступов.</p></sec><sec><title>ГЕТЕРОТОПИИ СЕРОГО ВЕЩЕСТВА ГОЛОВНОГО МОЗГА / GRAY MATTER HETEROTOPIA</title><p>Гетеротопии – это АРК, возникающие вследствие нарушенной миграции нейроцитов [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit21">21</xref>]. При данном виде патологии наблюдается атипичная локализация группы нейроцитов в белом веществе или вдоль стенок боковых желудочков мозга [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit21">21</xref>]. Гетеротопии проявляются конгломератами серого вещества и характеризуются изоинтенсивным сигналом во всех режимах МРТ [<xref ref-type="bibr" rid="cit22">22</xref>]. Гетеротопические очаги серого вещества не накапливают контрастное вещество и не содержат кальцификатов [<xref ref-type="bibr" rid="cit23">23</xref>].</p><p>В зависимости от локализации выделяют перивентрикулярные (субэпендимальные), субкортикальные, лептоменингеальные (маргинальные) гетеротопии серого вещества [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit11">11</xref>]. Если гетеротопия представлена в виде скопления нейроцитов, то она определяется как узловая, если в виде полос серого вещества вдоль стенок боковых желудочков – как ламинарная [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit24">24</xref>].</p></sec><sec><title>Перивентрикулярная узловая гетеротопия / Periventricular nodular heterotopia</title><p>Перивентрикулярная гетеротопия является наиболее распространенным типом гетеротопий и чаще всего представлена скоплением серого вещества в виде узлов, выстилающих стенку бокового желудочка. Картина гетеротопии зависит от количества, локализации, величины и формы узлов [<xref ref-type="bibr" rid="cit1">1</xref>]. Число узлов может варьироваться от одного до нескольких, они могут располагаться изолированно или сливаться.</p><p>Распространенной является форма диффузной перивентрикулярной узловой гетеротопии, возникающей вследствие мутаций в X-сцепленном гене филамина А (FLNA) [<xref ref-type="bibr" rid="cit25">25</xref>]. Эта форма описана преимущественно у женщин. Течение заболевания у мужчин более вариабельно. Когнитивные нарушения у мужчин встречаются чаще, чем у женщин [<xref ref-type="bibr" rid="cit26">26</xref>]. Описана редкая аутосомно-рецессивная форма, ассоциированная с врожденной микроцефалией и двусторонней перивентрикулярной узловой гетеротопией [<xref ref-type="bibr" rid="cit27">27</xref>]. У детей отмечены выраженные нарушения в развитии.</p><p>Другие генетические формы перивентрикулярной узловой гетеротопии ассоциированы с хромосомами 5p15, 5q14.3, 6p25, 6q27 и 7q1123 [28–32]. Однако предполагаемый ген, являющийся причиной гетеротопии, идентифицирован только для формы, связанной с хромосомой 6q27 [<xref ref-type="bibr" rid="cit33">33</xref>]. Гены ARFGEF2 и FLNA, ассоциированные с перивентрикулярной узловой гетеротопией, регулируют связывание актина, транспортировку везикул, адгезию клеток и функцию радиальных глиальных клеток [27, 34]. Мутации этих генов вызывают аномалии эпендимы – эпителия, выстилающего стенки желудочков мозга [<xref ref-type="bibr" rid="cit35">35</xref>].</p><p>У пациентов с классической Х-сцепленной формой гетеротопии в большинстве случаев выявляют билатеральную сливающуюся перивентрикулярную узловую гетеротопию, которая не затрагивает височные рога боковых желудочков мозга. При этом может наблюдаться гипоплазия червя мозжечка [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>У пациентов с аутосомно-рецессивной билатеральной перивентрикулярной узловой гетеротопией обнаруживают тяжелую врожденную микроцефалию и истонченную кору мозга с аномальными извилинами [<xref ref-type="bibr" rid="cit27">27</xref>].</p><p>Перивентрикулярная узловая гетеротопия при синдроме поражения задних отделов полушарий мозга ограничена треугольниками в области височных и затылочных рогов боковых желудочков и ассоциируется с полимикрогирией, гипоплазией гиппокампа и мозжечка или гидроцефалией [<xref ref-type="bibr" rid="cit36">36</xref>].</p><p>В редких случаях перивентрикулярная гетеротопия может проявляться в виде толстых полос серого вещества вдоль стенок боковых желудочков (ламинарная гетеротопия) [<xref ref-type="bibr" rid="cit37">37</xref>].</p></sec><sec><title>Подкорковая гетеротопия / Subcortical heterotopia</title><p>Подкорковая гетеротопия характеризуется скоплением нейронов в белом веществе полушарий большого мозга. В ряде случаев гетеротопированные нейроциты распространяются от стенки бокового желудочка к коре мозга. «Трансмантийная» гетеротопия представляет собой тонкие полосы серого вещества, состоящие из колоннообразных кластеров нейроцитов или скоплений их по типу объемных образований, имеющих вид криволинейных закрученных узоров [<xref ref-type="bibr" rid="cit2">2</xref>]. Она иногда захватывает всю долю мозга и при нейровизуализации наблюдается феномен двойной доли мозга [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Выделена особая форма подкорковой гетеротопии, которая характеризуется возникновением гигантских извилистых полос серого вещества, симметрично охватывающих оба полушария мозга, и ассоциируется с агенезией мозолистого тела и диффузной полимикрогирией. Для описания этой формы был предложен термин «лентовидная гетеротопия серого вещества мозга». При расположении гетеротопированных нейронов в субкортикальных и глубоких отделах мозга создается иллюзия двойной коры [<xref ref-type="bibr" rid="cit38">38</xref>][<xref ref-type="bibr" rid="cit39">39</xref>]. На МРТ-изображениях визуализируются лентовидные полосы серого вещества, проходящие параллельно боковому желудочку и отделенные от коры и третьего желудочка мозга слоями белого вещества [<xref ref-type="bibr" rid="cit40">40</xref>].</p><p>Показано, что подкорковые гетеротопии могут вызываться мутациями в гене DCX (табл. 2) [<xref ref-type="bibr" rid="cit41">41</xref>]. В этих случаях они наблюдаются у женщин, т.к. ген локализуется на Х-хромосоме. У 80–90% больных с перивентрикулярной узловой и подкорковой гетеротопиями наблюдается эпилепсия [<xref ref-type="bibr" rid="cit5">5</xref>]. При подкорковой ламинарной гетеротопии регистрируются резко выраженные неврологические и когнитивные нарушения. Эпилепсия возникает у 50% пациентов с синдромом двойной коры [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit44">44</xref>].</p><p>Эпилепсия при гетеротопиях может дебютировать в любом возрасте (как у детей, так и у взрослых). Данные об эффективности медикаментозной терапии противоречивы. Встречаются как поддающиеся лечению, так и фармакорезистентные формы [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>Установление локализации эпилептогенной области представляет большие трудности в связи с образованием эпилептогенных сетей [<xref ref-type="bibr" rid="cit45">45</xref>]. Гетеротопические области, выявляемые при иктальной однофотонной эмиссионной компьютерной томографии, служат одной из наиболее частых причин рефрактерных эпилепсий [<xref ref-type="bibr" rid="cit46">46</xref>]. Исследования с помощью глубинных электродов показали, что эпилептическая активность может регистрироваться как в области гетеротопии и коре мозга [<xref ref-type="bibr" rid="cit45">45</xref>], так и лишь в узле гетеротопии [<xref ref-type="bibr" rid="cit47">47</xref>]. При гетеротопиях серого вещества, распространяющихся на обширные области, в случаях микроцефалии и других АРК вероятность возникновения когнитивных, неврологических и психических нарушений возрастает в значительной степени.</p></sec><sec><title>ФОКАЛЬНАЯ КОРКОВАЯ ДИСПЛАЗИЯ / FOCAL CORTICAL DYSPLASIA</title><p>Гистопатологическая классификация ФКД Международной Противоэпилептической Лиги (англ. ФКД Iа характеризуется множеством микроколонок, а также наличием гетеротопированных нейроцитов с локализацией в белом веществе и инвазией U-волокон [<xref ref-type="bibr" rid="cit49">49</xref>][<xref ref-type="bibr" rid="cit50">50</xref>]. Обе характеристики могут быть обнаружены с помощью методов иммуногистохимии: микроколоны – с помощью антител к NeuN, гетеротопированные нейроны – с помощью MAP2 [<xref ref-type="bibr" rid="cit51">51</xref>]. ФКД Ib представлена аномальной организацией шести гистологических слоев коры, поэтому данный вариант называют нарушением горизонтальной архитектоники. ФКД Ic сочетает в себе признаки нарушений горизонтальной и вертикальной организации [<xref ref-type="bibr" rid="cit48">48</xref>].</p><p>ФКД II типа наиболее часто встречается у пациентов, перенесших хирургическое лечение эпилепсии, и составляет около 9% всех случаев ФКД, причем в 51% случаев аномалия локализуется в лобной доле [<xref ref-type="bibr" rid="cit52">52</xref>]. Основная характеристика ФКД II – наличие видоизмененных гигантских нейронов (дисморфических нейронов), значительно более крупных, чем пирамидные клетки. Глиальные клетки в структуре аномалии также увеличены, однако этот признак не является необходимым для диагностики. Специфический признак ФКД IIb и отличие ее от ФКД IIa – наличие клеток, подвергшихся баллонной дистрофии, которые одновременно экспрессируют нейрональные и глиальные белки. Баллонные клетки часто окружают область, в которой сгруппированы дисморфические нейроциты. При этом дисморфические нейроциты являются источником патологической активности, а баллонные клетки – нет. ФКД II типа также характеризуется дезорганизацией шести слоев коры, причем клетки области аномалии расположены менее плотно [<xref ref-type="bibr" rid="cit51">51</xref>]. В 60% случаев ФКД II типа ассоциирована с соматической мутацией в генах белков метаболического пути mTOR, наиболее часто в гене MTOR при ФКД IIb [<xref ref-type="bibr" rid="cit53">53</xref>]. Герминальные мутации с потерей функции описаны в основном при ФКД IIa, причем характерен феномен двойного удара: сочетание герминальной мутации с внезапно возникшей соматической приводит к потере защитной функции гетерозиготного аллеля и проявлению патологического признака [54–56].</p><p>ФКД III типа представляет собой патологическую организацию неокортекса в сочетании с другими эпилептогенными структурными нарушениями: гиппокампальным склерозом (ФКД IIIа), опухолью головного мозга (ФКД IIIb), сосудистой мальформацией (ФКД IIIc), ишемическими, геморрагическими или воспалительными изменениями, возникшими в раннем возрасте (ФКД IIId). При ФКД IIIа у пациентов со склерозом гиппокампа определяется утрата нейронов 2-го и 3-го слоев коры [<xref ref-type="bibr" rid="cit57">57</xref>]. Диагностика ФКД IIIb является редкой и требует иммуногистохимической верификации с целью исключения инфильтрации неокортекса глионейрональной опухолью [<xref ref-type="bibr" rid="cit58">58</xref>][<xref ref-type="bibr" rid="cit59">59</xref>]. При ФКД IIIc часто обнаруживают сочетание изменений горизонтальной и вертикальной организации коры (синдром Стерджа–Вебера) [<xref ref-type="bibr" rid="cit60">60</xref>][<xref ref-type="bibr" rid="cit61">61</xref>]. ФКД IIId, при которой утрачивается 4-й слой коры, обнаруживается преимущественно у мальчиков с перинатальным гипоксическим поражением затылочной доли [<xref ref-type="bibr" rid="cit62">62</xref>].</p><p>Малые мальформации развития коры мозга (англ. mild cortical developmental malformation, mCDM) микроскопически характеризуются расположением гетеротопированных нейроцитов в белом веществе при отсутствии других значимых изменений. При mMCD количество нейронов в 1 мм2 обычно составляет более 30, в противном случае аномалия не является mCDM, что было подтверждено в экспериментах с автоматическим подсчетом NeuN-позитивных нейронов [<xref ref-type="bibr" rid="cit63">63</xref>].</p><p>Кроме того, mCDM с гиперплазией олигодендроглии (англ. mild malformation of cortical development with oligodendroglial hyperplasia in epilapsy, MOGHE) характеризуются также увеличением количества олигодендроглиоцитов, которое составляет более 2200 клеток на 1 мм2 [<xref ref-type="bibr" rid="cit64">64</xref>][<xref ref-type="bibr" rid="cit65">65</xref>]. Примерно 45–100% пациентов с MOGHE имеют соматическую мутацию в гене SLC35A2 [<xref ref-type="bibr" rid="cit66">66</xref>][<xref ref-type="bibr" rid="cit67">67</xref>]. Показано, что клетки мозговой ткани с этой мутацией отличаются патологическим паттерном гликозилирования [<xref ref-type="bibr" rid="cit68">68</xref>].</p><p>Термин «отсутствие определенного типа ФКД в образце» рекомендуется использовать в случаях, если изменения архитектоники ткани гистологического образца не соответствуют характеристикам полей Бродмана и критериям одного из типов ФКД (для исключения ФКД необходимо проведение окрашивания на NeuN и MAP2) [<xref ref-type="bibr" rid="cit48">48</xref>].</p><p>Для выявления герминальных и соматических мутаций у пациентов с ФКД рекомендуется применение методов ДНК-диагностики. Было выявлено девять генов, мутации в которых приводят к формированию канонической ФКД II типа: AKT3, DEPDC5, MTOR, NPLR2, NPLR3, PIK3CA, RHEB, TSC1 и TSC2. Ген SLC35A2 рекомендуется проанализировать с целью дифференциальной диагностики MOGHE и ФКД Iа [<xref ref-type="bibr" rid="cit48">48</xref>][<xref ref-type="bibr" rid="cit66">66</xref>][<xref ref-type="bibr" rid="cit67">67</xref>]. При описании результатов генетического исследования должны быть указаны значение мутации (усиление или ослабление функции продукта гена), ее локализация, тип биоматериала и методика [<xref ref-type="bibr" rid="cit48">48</xref>].</p><p>МРТ представляет собой важный этап диагностики при обследовании пациентов с фокальной эпилепсией и подозрением на ФКД [<xref ref-type="bibr" rid="cit69">69</xref>][<xref ref-type="bibr" rid="cit70">70</xref>]. Так, например, «трансмантийная гетеротопия» является патогномоничным признаком ФКД IIb. Современная классификация АКР отражает нейровизуа- лизационные и гистопатологические изменения, молекулярные и генетические нарушения. Достижения в области методов генетического секвенирования позволят в будущем выявить генетические причины большинства случаев АРК.
