Данный обзор  посвящен аутоиммунной эпилепсии с учетом последних достижений в области изучения патофизиологии  заболевания. Приведены определения, рассмотрены патогенетические механизмы, особенности  нейрональных антител и клинической картины АИЭ в зависимости  от типа аутоантител. Обоснована необходимость  регулярного наблюдения пациентов с АИЭ, предпочтительно эпилептологом совместно  с нейроиммунологом.  При длительном наблюдении у некоторых больных сохраняется хроническая фармакорезистентная  эпилепсия, несмотря на агрессивную  иммунотерапию и прием противоэпилептических препаратов. При более глубоком понимании механизмов антителоопосредованных  и аутоантигенспецифических Т-клеточно-опосредованных синдромов  АИЭ использование противоэпилептических препаратов и иммунотерапии может быть дополнительно оптимизировано.</p></abstract><trans-abstract xml:lang="en"><p>Investigation of autoimmune epilepsy (AIE) has been attracting increasingly more attention due to discovery of neuronal antibodies and improved understanding of the mechanisms related to such immune-mediated syndromes. The review is aimed at autoimmune epilepsy taking into account up-to-date advances  in exploring its pathophysiology. Definitions related to this issue are outlined, and pathogenetic mechanisms, features of antineuronal antibodies as well as AIE clinical picture based on type of autoantibodies, are considered. The necessity of regular monitoring patients with AIE is indicated, preferably by an epileptologist  together  with a neuroimmunologist.  With prolonged  follow-up,  chronic  pharmacoresistant  epilepsy persists in some  patients, despite aggressive immunotherapy and antiepileptic drugs. With a deeper understanding of the mechanisms of antibody-mediated and autoantigen-specific T-cell-mediated AIE syndromes, the use of antiepileptic drugs and immunotherapy can be further optimized.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>Аутоиммунная эпилепсия</kwd><kwd>дефиниция</kwd><kwd>патофизиология</kwd><kwd>нейрональные антитела</kwd><kwd>клиническая картина</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Autoimmune epilepsy</kwd><kwd>definition</kwd><kwd>pathophysiology</kwd><kwd>antineuronal antibodies</kwd><kwd>clinical picture</kwd></kwd-group></article-meta></front><body><sec><title>ВВЕДЕНИЕ / INTRODUCTION</title><p>Аутоиммунная эпилепсия (АИЭ) была выделена Международной Противоэпилептической Лигой (англ. International League Against Epilepsy, ILAE) как самостоятельная форма заболевания в последней классификации эпилепсии 2017 г. АИЭ – результат прямого иммунного расстройства, при котором эпилептические приступы являются основным его симптомом [<xref ref-type="bibr" rid="cit1">1</xref>]. Обособлению данной формы заболевания способствовало открытие в течение последних десятилетий нейронных аутоантител, нацеленных на поверхностные или внутриклеточные антигены, и изучение иммуноопосредованных процессов в нервной ткани [2–6]. Особый интерес АИЭ представляет в настоящее время, учитывая зарегистрированные случаи аутоиммунного лимбического энцефалита с развитием эпилептических приступов у пациентов с COVID-19 [7–10].</p><p>Точная частота встречаемости данной формы эпилепсии неизвестна ввиду отсутствия популяционных исследований. Частота АИЭ у взрослых, по данным ранее проведенных исследований, может составлять около 5–7% от всех форм эпилепсии [<xref ref-type="bibr" rid="cit3">3</xref>][11–14]. Показано, что частота определения нейронспецифических антител среди пациентов без генетической, структурной или метаболической этиологии эпилепсии составляет 15% [<xref ref-type="bibr" rid="cit13">13</xref>], у взрослых больных эпилепсией неуточненной этиологии – 20% [<xref ref-type="bibr" rid="cit3">3</xref>]. В исследовании, проведенном в США в 2018 г., представлен показатель заболеваемости аутоиммунным энцефалитом 0,8 случая на 100 тыс. человек в год, а распространенности – 13,7 случая на 100 тыс. человек [<xref ref-type="bibr" rid="cit14">14</xref>].</p><p>У детей с впервые возникшей эпилепсией показано наличие аутоантител примерно в 10% случаев по результатам исследований, проведенных в Европе и Австралии [<xref ref-type="bibr" rid="cit15">15</xref>][<xref ref-type="bibr" rid="cit16">16</xref>].</p><p>Эпилепсия неуточненной этиологии составляет 1/3 всех эпилепсий среди взрослых [<xref ref-type="bibr" rid="cit12">12</xref>].</p><p>Значительную роль в понимании этиопатогенеза АИЭ сыграло открытие нейронспецифических антител. В настоящее время выделяют два триггера формирования аутоиммунных нарушений. Иммунологическим триггером может являться объемное образование [<xref ref-type="bibr" rid="cit17">17</xref>][<xref ref-type="bibr" rid="cit18">18</xref>]. В этом случае онконейрональный антиген, экспрессируемый клетками опухоли, приводит к направленному иммунному ответу, способствующему дисфункции нейронов. Некоторые антитела являются специфичными для отдельных типов опухоли [<xref ref-type="bibr" rid="cit19">19</xref>]. Другим потенциальным триггером является инфекция [<xref ref-type="bibr" rid="cit20">20</xref>].</p><p>Предложены различные механизмы, включая молекулярную мимикрию, когда структурное и/или аминокислотное сходство последовательности патогена с аутоантигеном приводит к перекрестному реактивному иммунному ответу. Повреждение клеток инфекционным агентом может повлечь за собой высвобождение предполагаемого аутоантигена, вызывая аутоиммунный ответ. И наконец, инфекция может привести к активации аутореактивных лимфоцитов и антигенпрезентирующих клеток, что вызовет иммунный ответ против аутоантигенов (рис. 1). Примером инфекционного триггера, приводящего к аутоиммунному ответу, является NMDAR1-аутоиммунный энцефалит, развивающийся после вируса простого герпеса (ВПГ) [<xref ref-type="bibr" rid="cit21">21</xref>].</p><fig id="fig-1"><caption><p>Рисунок 1. При этих расстройствах антигенами-мишенями являются ядерные или цитоплазматические белки, такие как Hu, Yo и Ma2. Вторая группа заболеваний головного мозга, связанных с антителами, включает антитела, которые нацелены на внутриклеточные синаптические белки, такие как декарбоксилаза глутаминовой кислоты 65 кДа (GAD65) и амфифизин.</p><p>Аутоантигены при иммуноопосредованной эпилепсии могут быть разделены на основе их клеточной локализации на поверхностные или внутриклеточные белки, обладающие различными иммуноопосредованными механизмами [<xref ref-type="bibr" rid="cit19">19</xref>][<xref ref-type="bibr" rid="cit22">22</xref>].</p><p>Так, ядерные или цитоплазматические белки (например, Hu, Yo и Ma2), будучи антигенами-мишенями, имеют ограниченный доступ для антител. Соответственно, антитела к ним не являются непосредственно патогенными, а указывают на опосредованный Т-клетками иммунный ответ против соответствующих нейрональных антигенов (см. рис. 1) [<xref ref-type="bibr" rid="cit22">22</xref>]. Повреждение нейронов обусловлено CD8+ цитотоксическим Т-клеточным ответом. Эти антитела и нейротоксические Т-клетки могут иметь одну и ту же аутоантигенспецифичность, например паранеопластический синдром Anti-Yo (CDR2) [<xref ref-type="bibr" rid="cit23">23</xref>]. Ответ на терапию обычно бывает слабым при неврологических расстройствах, связанных с этими аутоантителами, – возможно, из-за необратимой гибели нейронов.</p><p>Вторая группа – антитела, нацеленные на внутриклеточные синаптические белки декарбоксилаза глутаминовой кислоты 65 кДа (GAD65) и амфифизин. Эти антигены могут быть уязвимы для опосредованного антителами разрушения во время слияния и обратного захвата синаптических везикул, но вопрос о том, являются ли опосредованные Т-клетками патогенные механизмы более важными, чем опосредованные антителами, остается предметом дискуссий [<xref ref-type="bibr" rid="cit22">22</xref>].</p><p>Третья группа включает аутоантитела, нацеленые на рецепторы клеточной поверхности или синаптические белки, играющие прямую патогенную роль. Например, антитела к богатому лейцином инактивируемому глиомой протеину (англ. leucine-rich glioma-inactivated protein 1, 1LGI1), комплексу вольтаж-зависимых калиевых каналов (англ. voltage-gate potassium channel complex, VGKCс), рецептору N-метил-D-аспартата (англ. N-methyl-D-aspartate receptor, NMDAR), рецептору гамма-аминомасляной кислоты (англ. gammaaminobutyric acid receptor, GABAR) и рецептору альфаамино-3-гидрокси-5-метил-4-изоксазолпропионовой кислоты (англ. alpha-amino-3-hydroxy-5-methyl-4isoxazolepropionic acid receptor, AMPAR) [<xref ref-type="bibr" rid="cit22">22</xref>].</p><p>Заболевания, связанные с антителами против антигенов клеточной поверхности, отличаются от связанных с внутриклеточными антигенами по нескольким важным аспектам. Во-первых, антигены-мишени на клеточной поверхности разрушаются антителами. Во-вторых, связь со злокачественными новообразованиями менее последовательна. В-третьих, симптомы чаще можно обратить вспять с помощью лечения. Наконец, симптомы связаны с нарушением действия антигена-мишени. Было доказано, что иммуноглобулины класса G (англ. immunoglobulin G, IgG) к LGI1 вызывают патогенный эффект из-за дефицита ионных каналов. LGI1 IgG приводит к нарушению взаимодействия LGI1 с доменом дезинтегрина и металлопротеазы (англ. a disintegrin and metalloprotease domain, ADAM22), которое снижает синаптическую функцию AMPAR, впоследствии нарушая приток кальция [<xref ref-type="bibr" rid="cit24">24</xref>][<xref ref-type="bibr" rid="cit25">25</xref>]. С другой стороны, NMDAR IgG связывается с областью субъединицы GluN1 NMDAR. Это нарушает взаимодействие между NMDAR и рецептором эфрина типа B2 и инициирует интернализацию NMDAR, способствующую нейрональной дисфункции [<xref ref-type="bibr" rid="cit26">26</xref>]. Процесс интернализации обратим, и было продемонстрировано хорошее восстановление при удалении NMDAR IgG [<xref ref-type="bibr" rid="cit27">27</xref>].</p><p>Показано, что роль дисфункции гематоэнцефалического барьера, инфильтрации миелоидных клеток и инициации провоспалительных каскадов в эпилептогенезе имеет важное значение в моделях животных и людей с эпилепсией. Эпилептогенез при АИЭ, вероятно, возникает в результате синергического эффекта как врожденного нейровоспаления, так и вызванной антителами гипервозбудимости. В совокупности эти процессы приводят к стойким лекарственно-устойчивым приступам, которые способствуют заболеванию, приводящему к АИЭ [<xref ref-type="bibr" rid="cit28">28</xref>].</p><p>Однако в дальнейшем в научном сообществе возникла активная дискуссия о дефиниции АИЭ.</p><p>Целевая группа по аутоиммунитету и воспалению ILAE в 2020 г. предложила концептуальные определения приступов, возникающих при аутоиммунных нарушениях, соответствующие современным определениям и концепциям эпилепсии [<xref ref-type="bibr" rid="cit29">29</xref>]. Так, термин «острые симптоматические приступы, вторичные по отношению к аутоиммунному энцефалиту» предложен для обозначения приступов, возникающих в активной фазе иммуноопосредованного энцефалита. А дефиниция «аутоиммунно-ассоциированная эпилепсия» предназначена для обозначения хронических эпилептических приступов, определяемых как вторичные по отношению к аутоиммунным заболеваниям головного мозга. Аутоиммунно-ассоциированная эпилепсия может быть следствием продолжающихся аутоиммунных нарушений, а также связанных с ними структурных изменений головного мозга. Различие между острыми симптоматическими приступами, вторичными по отношению к аутоиммунному энцефалиту, и аутоиммунно-ассоциированной эпилепсией имеет клиническое и терапевтическое значение [<xref ref-type="bibr" rid="cit29">29</xref>]. В отличие от больных с острыми симптоматическими эпилептическими приступами, у пациентов с иммуноопосредованными заболеваниями головного мозга регистрируются хронические приступы, устойчивые как к противоэпилептической терапии, так и к иммунотерапии [30–32].</p><p>Эти данные свидетельствуют о том, что цитотоксические Т-клетки играют более заметную роль в патогенезе, приводя к гибели нейронов, а нейронные аутоантитела присутствуют скорее как побочный продукт иммунного ответа и не играют прямой патогенной роли [33–35].</p><p>Таким образом, продолжение эпилептических приступов, несмотря на иммунотерапию, предполагает устойчивую предрасположенность, что соответствует современному концептуальному определению эпилепсии. Кроме того, низкая частота ремиссии приступов свидетельствует о том, что риск их развития в течение следующих 10 лет очень высок. Эти условия соответствуют концептуальным и практическим определениям эпилепсии [<xref ref-type="bibr" rid="cit36">36</xref>][<xref ref-type="bibr" rid="cit37">37</xref>].</p><p>Остаются нерешенными вопросы, касающиеся этиологической причины эпилептических приступов. Термин «аутоиммунная эпилепсия» подразумевает исключительно аутоиммунную этиологию. Однако у пациентов с АИЭ часто регистрируют структурные изменения, например склероз гиппокампа или мультифокальную потерю клеток коры головного мозга при глиозе, выявляемом при энцефалите Расмуссена [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit38">38</xref>].</p><p>Отмечено, что «иммунная» и «структурная» этиологии могут сосуществовать у одного и того же пациента, и обе, вероятно, играют определенную роль в возникновении приступов. Некоторые наблюдения подтверждают концепцию структурной (постэнцефалитной) эпилепсии [<xref ref-type="bibr" rid="cit39">39</xref>]. Энцефалиты, опосредованные Т-клетками, приводят к потере нейронов и глиозу, что было продемонстрировано на примере энцефалита Расмуссена и связанного с ним нарушения тканей больших полушарий головного мозга [<xref ref-type="bibr" rid="cit35">35</xref>][<xref ref-type="bibr" rid="cit39">39</xref>][40–42].</p><p>Кроме того, гистопатологические исследования тканей, полученных после хирургического лечения эпилепсии у пациентов с нейронными антителами, показывают различную степень нейронной потери, включая склероз гиппокампа. Однако не все пациенты с антителоопосредованным энцефалитом имеют на магнитнорезонансной томограмме (МРТ) или при гистопатологическом исследовании признаки атрофии головного мозга. Действительно, структурные повреждения, приводящие к эпилепсии после энцефалита, могут быть микроскопическими и не всегда видны по данным нейровизуализации.</p><p>Предложенный термин «аутоиммунно-ассоциированная эпилепсия» подчеркивает, что иммунные факторы, постэнцефалитическое структурное повреждение или их комбинация могут способствовать хронической предрасположенности к приступам. Использование термина «аутоиммунная эпилепсия» подразумевает наличие активной воспалительной реакции и предполагает, что в качестве основного лечения должна рассматриваться иммунотерапия. Однако иммуномодифицирующая терапия у таких пациентов часто оказывается неэффективной. Менее определенный термин «аутоиммунно-ассоциированная эпилепсия» позволяет предположить присутствие других факторов, например структурных, которые могут играть определенную роль в возникновении эпилептических приступов. Эта концепция позволяет клиницисту продолжать лечение эпилепсии традиционными противоэпилептическими средствами и использовать хирургическое лечение при выявлении структурных изменений головного мозга [<xref ref-type="bibr" rid="cit38">38</xref>].</p><p>С другой стороны, структурные изменения головного мозга не всегда ассоциированы с возникновением эпилепсии. Действительно, атрофия гиппокампа является частым следствием энцефалита с LGI1-антителами, но ее наличие необязательно приводит к эпилепсии согласно предварительным лонгитюдным исследованиям [43–45]. Во-вторых, даже на поздних стадиях энцефалита Расмуссена может сохраняться минимальное Т-клеточное воспаление [<xref ref-type="bibr" rid="cit35">35</xref>]. Точно так же нет никаких доказательств прямой роли антител в патогенности или последовательном ответе на иммунотерапию. Так, гистопатологические доказательства воспаления выявлены при биопсии головного мозга и оперативном лечении эпилепсии у пациентов с онконевральными антителами к GAD65 [<xref ref-type="bibr" rid="cit33">33</xref>][<xref ref-type="bibr" rid="cit38">38</xref>]. Предполагается, что воспаление, опосредованное Т-клетками, может привести к длительным изменениям в головном мозге, способствующим спонтанным эпилептическим приступам [<xref ref-type="bibr" rid="cit46">46</xref>].</p><p>Рассмотрим клинические особенности иммуноопосредованных эпилептических приступов с учетом нейроспецифических антител (табл. 1). Приступы обычно устойчивы к противоэпилептическим препаратам на ранних стадиях заболевания [19,][47–50], а также в дебюте заболевания часто регистрируется эпилептический статус [<xref ref-type="bibr" rid="cit51">51</xref>][<xref ref-type="bibr" rid="cit52">52</xref>].</p><fig id="fig-2"><caption><p>Таблица 1. SE – status epilepticus; EEG – electroencephalography;PERM – progressive encephalomyelitis with rigidity and myoclonus;REM – rapid eye movement; NREM – non-rapid eye movement.</p></caption><graphic xlink:href="epilepsia-14-1-g002.jpeg"><uri content-type="original_file">https://cdn.elpub.ru/assets/journals/epilepsia/2022/1/PWX6xDxObbFTxcipTsii7x4x7qtozpCOij95V7VQ.jpeg</uri></graphic></fig><p>В когортах пациентов с различными типами аутоантител регистрируются несколько видов эпилептических приступов высокой частоты [<xref ref-type="bibr" rid="cit72">72</xref>][<xref ref-type="bibr" rid="cit85">85</xref>], короткая продолжительность приступов, отсутствие постприступной спутанности сознания, приступы перисильевой семиологии (например, смешанные ауры, клонии мимических мышц, сенсорная афазия) [<xref ref-type="bibr" rid="cit86">86</xref>][<xref ref-type="bibr" rid="cit87">87</xref>], склонность к билатеральным тонико-клоническим приступам (БТКП) в ночное время [<xref ref-type="bibr" rid="cit85">85</xref>].</p><p>У пациентов обычно отсутствуют другие типичные факторы риска развития эпилепсии, такие как фебрильные приступы или наследственный анамнез эпилепсии [<xref ref-type="bibr" rid="cit29">29</xref>].</p><p>Эпилептический статус в дебюте заболевания особенно часто встречается при энцефалите с антителами к рецепторам гамма-аминомасляной кислоты А и В (ГАМКAR и ГАМКBR) [87–90].</p><p>Когнитивные и поведенческие нарушения, как правило, сопровождают острые симптоматические приступы, вторичные по отношению к аутоиммунному энцефалиту, и включают нарушения памяти [<xref ref-type="bibr" rid="cit44">44</xref>], настроения, психоз и кататонию, как это наблюдается при анти-NMDAR-энцефалите [<xref ref-type="bibr" rid="cit91">91</xref>][<xref ref-type="bibr" rid="cit92">92</xref>]. Дизавтономные и двигательные нарушения, например орофациальная дистония, характерная для анти-NMDAR-энцефалита [<xref ref-type="bibr" rid="cit92">92</xref>], может быть ошибочно диагностирована как эпилептический приступ [<xref ref-type="bibr" rid="cit93">93</xref>].</p><p>Возникновению аутоиммунного энцефалита может предшествовать вирусный продром. Наиболее ярким примером является энцефалит, вызванный ВПГ, при котором энцефалит с NMDAR-антителами может возникнуть через несколько недель после первоначального разрешения [<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit94">94</xref>].</p><p>Наличие злокачественных новообразований в анамнезе, особенно за несколько лет до начала приступа, свидетельствует о возможности развития паранеопластического энцефалита [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit95">95</xref>][<xref ref-type="bibr" rid="cit96">96</xref>]. Фактором риска также может быть личный или семейный анамнез системных аутоиммунных расстройств [<xref ref-type="bibr" rid="cit48">48</xref>].</p></sec><sec><title>ОСОБЕННОСТИ КЛИНИЧЕСКОЙ КАРТИНЫ ИММУННОЙ ЭПИЛЕПСИИ В ЗАВИСИМОСТИ ОТ ТИПА АУТОАНТИТЕЛ / FEATURES OF IMMUNE EPILEPSY CLINICAL PICTURE DEPENDING ON THE TYPE OF AUTOANTIBODIES</title></sec><sec><title>Антитела к клеточной поверхности / Antibodies to cell surface</title><p>NMDAR IgG (антитела к N-метил-D-аспартатному рецептору)</p><p>Энцефалит с антителами к NMDAR обычно поражает молодых женщин с медианой возраста 22 года (диапазон от 2 мес до 85 лет) [<xref ref-type="bibr" rid="cit65">65</xref>]. Клиническая картина, как правило, начинается с приступа головной боли или лихорадки, за которым следуют психические проявления, включая галлюцинации, маниеподобные эпизоды, чередующиеся эпизоды крайнего возбуждения и кататонии. Затем развиваются судороги, энцефалопатия, орофациальная дискинезия, хореоатетоз и вегетативная дисфункция [<xref ref-type="bibr" rid="cit65">65</xref>]. Регистрируются фокальные немоторные приступы, которые могут прогрессировать до рефрактерного эпилептического статуса [<xref ref-type="bibr" rid="cit60">60</xref>][<xref ref-type="bibr" rid="cit97">97</xref>].</p><p>При отсутствии своевременного лечения возможно прогрессирование с развитием комы [<xref ref-type="bibr" rid="cit65">65</xref>].</p><p>Триггерный фактор может быть идентифицирован примерно в половине случаев. Двумя основными триггерами являются наличие тератомы яичников [<xref ref-type="bibr" rid="cit67">67</xref>] и энцефалит, вызванный ВПГ [<xref ref-type="bibr" rid="cit21">21</xref>]. Сообщалось, что примерно у 2/3 взрослых женщин в возрасте от 18 до 45 лет с энцефалитом с антителами к NMDAR имеется тератома яичников [<xref ref-type="bibr" rid="cit66">66</xref>]. Однако наличие этой опухоли крайне редко встречается у детей младше 12 лет или у пожилых людей (45 лет и старше) [<xref ref-type="bibr" rid="cit6">6</xref>][<xref ref-type="bibr" rid="cit65">65</xref>][<xref ref-type="bibr" rid="cit66">66</xref>]. Кроме того, проспективная оценка пациентов с ВПГ-энцефалитом показала, что в 17% этих случаев во время наблюдения развился энцефалит NMDAR. Еще у 3 больных в этой когорте при последующей оценке были выявлены иммуноглобулины позднего иммунного ответа к NMDAR без каких-либо клинических признаков аутоиммунного энцефалита [<xref ref-type="bibr" rid="cit21">21</xref>].</p><p>В единичных исследованиях представлены генетические предикторы развития энцефалита с антителами к NMDAR. Выявлена слабая ассоциация у взрослых пациентов – носителей аллеля B*07:02 лейкоцитарного антигена человека (англ. human leukocyte antigen, HLA), основного комплекса гистосовместимости (англ. histocompatibility complex, MHC) [<xref ref-type="bibr" rid="cit73">73</xref>].</p><p>LGI1 IgG (антитела к инактивированной лейцин-богатой глиоме 1)</p><p>Фациобрахиальные дистонические приступы ассоциированы с энцефалитом с антителами к LGI1 [73–75]. Другие известные типы фокальных приступов с сохранным сознанием, ассоциированных с LGI1-антителами, включают тепловые, сенсорные и пиломоторные приступы и могут наблюдаться при других лимбических энцефалитах и неэнцефалитических формах лимбических фокальных приступов [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit72">72</xref>][<xref ref-type="bibr" rid="cit76">76</xref>][<xref ref-type="bibr" rid="cit77">77</xref>]. Пароксизмальные приступы головокружения редки, но характерны для энцефалита, ассоциированного с LGI1-антителами [<xref ref-type="bibr" rid="cit78">78</xref>].</p><p>Иммуноглобулины класса G к LGI1 обычно ассоциируются с эпилептическими приступами и нарушением памяти (как правило, у пациентов старше 40 лет), но также описано несколько случаев заболевания в детском возрасте [<xref ref-type="bibr" rid="cit98">98</xref>]. Одним из характерных фенотипов, описанных среди взрослых пациентов, являются фациобрахиальные дистонические приступы. Это стереотипные короткие фокальные дистонические моторные приступы, возникающие несколько раз в день [<xref ref-type="bibr" rid="cit74">74</xref>]. Другая характерная семиология приступа – эпизоды односторонней пилоэрекции. Кроме того, описаны приступообразные головокружения [<xref ref-type="bibr" rid="cit78">78</xref>]. Эти головокружения или «внетелесные переживания» («выход из тела») могут предшествовать энцефалопатии на 2–12 мес. HLA-анализ LGI1-ассоциированного энцефалита показал сильную связь с 27 однонуклеотидными вариантами (ОНВ) HLA-II и гаплотипами MHC класса II DRB1*07:01, DQA1*02:01 и DQB1*02:02 [<xref ref-type="bibr" rid="cit99">99</xref>].</p><p>Небольшое число пациентов (около 2%) с антителами к VGKCc имеют сопутствующие IgG к контактин-ассоциированно-подобному белку-2 CASPR–2. У них чаще встречается поражение периферической нервной системы (нейромиотония, миокимия или дизавтономия). Однако у значительной части больных, особенно пожилых, могут быть сопутствующие эпилептические приступы или энцефалит. Выявление только IgG к VGKCс в отсутствие IgG к LGI1 и/или CASPR-2 не может являться специфическим биомаркером аутоиммунитета [<xref ref-type="bibr" rid="cit43">43</xref>][<xref ref-type="bibr" rid="cit79">79</xref>].</p><p>AMPA IgG (антитела к α-амино-3-гидрокси-5-метил-4изоксазолпропионовой кислоте)</p><p>Антитела к рецептору AMPA направлены против субъединиц GluR1 и GluR2, вызывая снижение регуляции рецептора и уменьшение его синаптической кластеризации. Средний возраст начала заболевания составляет около 60 лет [<xref ref-type="bibr" rid="cit66">66</xref>]. AMPA-ассоциированный энцефалит обычно сопровождается классическими симптомами лимбического энцефалита (антероградное снижение памяти, ретроградная амнезия, изменения настроения и височные приступы). Недавние исследования подтвердили прямую патогенность, опосредованную антителами [<xref ref-type="bibr" rid="cit67">67</xref>][<xref ref-type="bibr" rid="cit68">68</xref>]. Две трети пациентов имеют первичную злокачественную опухоль, главным образом мелкоклеточный рак легких и тимому [<xref ref-type="bibr" rid="cit100">100</xref>]. У части больных может быть рефрактерное течение, несмотря на агрессивную иммунотерапию, и прогрессирующая диффузная атрофия коры головного мозга [<xref ref-type="bibr" rid="cit73">73</xref>][<xref ref-type="bibr" rid="cit68">68</xref>][<xref ref-type="bibr" rid="cit69">69</xref>].</p><p>ГАМКBR IgG (антитела к рецепторам типа В гамма-аминомасляной кислоты)</p><p>Рецепторы ГАМК типа В (GABAB1) связаны с G-белковыми калиевыми каналами. GABAB1рецепторный энцефалит обычно проявляется рефрактерным бессудорожным эпилептическим статусом [<xref ref-type="bibr" rid="cit66">66</xref>]. Большинство пациентов – пожилые мужчины (средний возраст 61 год) [<xref ref-type="bibr" rid="cit6">6</xref>]. В 50–60% случаев выявляется злокачественное новообразование, чаще всего мелкоклеточный рак легких.</p><p>ГАМКAR IgG (антитела к рецепторам типа А гамма-аминомасляной кислоты)</p><p>С ГАМК-рецепторным энцефалитом ассоциированы молниеносный энцефалит и рефрактерные эпилептические приступы или эпилептический статус. Такие пациенты на МРТ имеют характерные мультифокальные асимметричные кортикальные и подкорковые зоны, гиперинтенсивные в T2/FLAIR режимах [<xref ref-type="bibr" rid="cit60">60</xref>]. Возраст дебюта заболевания, как правило, меньше, чем у пациентов с ГАМК-В-энцефалитом (средний возраст 40 лет) [<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>DPPX IgG (антитела к дипептидил-пептидазоподобному белку-6)</p><p>Дипептидил-пептидазоподобный белок 6 (англ. dipeptidyl-peptidase-like protein, DPPX) является вспомогательной субъединицей клеточной поверхности семейства калиевых каналов Kv4.2. Пациенты с DPPXантителоассоциированным энцефалитом также могут иметь в клинической картине нарушение кинетики желудочно-кишечного тракта, потерю веса, когнитивную дисфункцию, делирий, гиперэкплексию, миоклонус, парасомнии, а иногда и прогрессирующий энцефаломиелит с ригидностью и миоклониями [<xref ref-type="bibr" rid="cit66">66</xref>][<xref ref-type="bibr" rid="cit80">80</xref>].</p><p>mGlur5 IgG (антитела к метаботропному глутаматному рецептору-5)</p><p>В 1982 г. I. Carr описал нейропсихиатрический синдром с нарушением поведения, депрессией, амнезией и галлюцинациями у своей 15,5-летней дочери с лимфомой Ходжкина. Она прошла курс лечения, что привело к значительному неврологическому улучшению. Автор назвал эту патологию «синдромом Офелии» в честь персонажа из пьесы Шекспира [<xref ref-type="bibr" rid="cit101">101</xref>]. Он предположил, что паранеопластический механизм опосредован циркулирующей нейромедиаторной молекулой, секретируемой опухолью. В 2011 г. IgG к метаботропному глутаматному рецептору 5 (mGlur5) был обнаружен у 2 пациентов с клиническими проявлениями, соответствующими «синдрому Офелии» (подострое начало энцефалопатии, изменение настроения, эпилептические приступы и лимфома Ходжкина) [<xref ref-type="bibr" rid="cit102">102</xref>]. В недавнем исследовании 11 пациентов с выявленными IgG к mGlur5 у 5 больных была обнаружена лимфома Ходжкина и у 1 – мелкоклеточный рак легкого [<xref ref-type="bibr" rid="cit70">70</xref>]. Общими клиническими проявлениями были нейропсихиатрические нарушения, расстройство движения, нарушение сна и эпилептические приступы. У 5 из 11 пациентов на МРТ головного мозга было выявлено поражение лимбической системы, а у 4 – изменения в экстралимбической области. У 2 больных эпилептический статус был первым клиническим признаком.</p><p>D2R IgG (антитела к поверхностному рецептору дофамина-2)</p><p>D2R – важнейший рецептор, регулирующий дофаминергическую нейротрансмиссию. R.C. Dale et al. (2012 г.) [<xref ref-type="bibr" rid="cit71">71</xref>] в исследовании детей и подростков с энцефалитом базальных ганглиев и другими предполагаемыми аутоиммунными нарушениями базальных ганглиев выявили повышенный уровень IgG к D2R у 12 из 17 пациентов по сравнению с контролем. Появление симптомов часто происходило в постинфекционных или поствакцинальных условиях. Пять из 12 детей имели клинический анамнез и серологические данные, свидетельствующие о предшествующей инфекции β-гемолитическим стрептококком (n=3), микоплазменной пневмонией (n=1) и энтеровирусом (n=1). Симптомы в дебюте заболевания варьировали, но вялость, психические симптомы, патологические движения или нарушение походки были типичными. В клиническом синдроме преобладал целый спектр двигательных расстройств, включая дистонию, паркинсонизм и хорею. Многие пациенты с дистонией имели грубый тремор. Окулогирные кризы имели место у 2 больных с дистонией или паркинсонизмом, а «трепетание» глаз зарегистрировано у 1 ребенка с хореей и атаксией.</p><p>У 9 из 12 пациентов наблюдались психические расстройства, в частности возбуждение, эмоциональная лабильность, тревожность и психотические симптомы. Нарушение сна, вялость, сонливость, дисфункция ствола головного мозга, эпилептические приступы и атаксия возникали реже. У половины больных по данным нейровизуализации структурные изменения головного мозга отсутствовали. У части пациентов выявлялись воспалительные изменения, локализующиеся в базальных ганглиях и структурах ствола головного мозга. Эпилептические приступы регистрировались в 2 случаях. Электроэнцефалограмма (ЭЭГ) была в пределах возрастной нормы либо показывала неспецифическое замедление, совместимое с энцефалопатией. Ни у одного пациента не было выявлено эпилептиформной активности по данным ЭЭГ [<xref ref-type="bibr" rid="cit71">71</xref>].</p><p>GlyR IgG (антитела к глициновому рецептору)</p><p>Антитела к глициновым рецепторам (GlyR) тесно связаны с заболеваниями спинного мозга и ствола головного мозга. У большинства пациентов наблюдается прогрессирующий энцефаломиелит с ригидностью и миоклонусом. A. Carvajal-González et al. (2014 г.) идентифицировали 52 пациента с положительными антителами к GlyR и сопоставили клинические особенности. У 33 больных был классифицирован прогрессирующий энцефаломиелит с ригидностью и миоклонусом, у 2 – синдром ригидного человека; у 5 – лимбический энцефалит и эпилептическая энцефалопатия, у 2 – нарушения ствола мозга, у 2 – демиелинизирующая оптическая невропатия, у 1 – неуточненный диагноз [<xref ref-type="bibr" rid="cit81">81</xref>].</p></sec><sec><title>Антитела к внутриклеточным структурам / Antibodies to intracellular structures</title><p>GAD65 IgG (антитела к декарбоксилазе глутаминовой кислоты)</p><p>Антитела GAD65 (титры сыворотки более 20 нмоль/л или обнаружение в ликворе) ассоциированы с различными аутоиммунными неврологическими заболеваниями, включая аутоиммунную эпилепсию, синдром ригидности, мозжечковую атаксию, лимбический энцефалит и прогрессирующий энцефаломиелит с ригидностью и миоклонией [<xref ref-type="bibr" rid="cit30">30</xref>][<xref ref-type="bibr" rid="cit61">61</xref>][<xref ref-type="bibr" rid="cit62">62</xref>][<xref ref-type="bibr" rid="cit103">103</xref>]. Женщины страдают чаще, чем мужчины, средний возраст начала симптомов составляет 30 лет (диапазон 5–80 лет) [<xref ref-type="bibr" rid="cit60">60</xref>]. В исследовании 112 пациентов с фокальной эпилепсией неуточненной этиологии с дебютом во взрослом возрасте у 5,4% были обнаружены высокие титры антител к GAD65 (более 1000 ЕД/мл) [<xref ref-type="bibr" rid="cit31">31</xref>]. Обычно больные с антителами GAD65 (титры сыворотки более 20 нмоль/л или обнаружение в ликворе) рефрактерны к терапии [<xref ref-type="bibr" rid="cit30">30</xref>][<xref ref-type="bibr" rid="cit31">31</xref>]. Постулируется, что рефрактерная природа заболевания вторична по отношению к клеточно-опосредованной цитотоксичности, а не к прямому патогенезу, опосредованному антителами.</p><p>Аnti-Hu (АNNA-1) IgG (нейрональные ядерные антитела типа 1)</p><p>Белки Hu, особенно HuD, имеют важное значение для регуляции нейрональной РНК, регуляции клеточного цикла и развития клеток [<xref ref-type="bibr" rid="cit53">53</xref>][<xref ref-type="bibr" rid="cit54">54</xref>]. У пациентов с IgG к АNNA-1 присутствуют различные проявления со стороны центральной и периферической нервной системы. Выявление IgG к ANNA-1 ассоциировано с мелкоклеточным раком (81%). Считается, что HuD-специфические Т-клетки играют важную роль в патогенности паранеопластического синдрома [<xref ref-type="bibr" rid="cit55">55</xref>]. Признаками аутоиммунитета с ANNA-1 являются сенсорная нейропатия и вегетативная дисфункция, особенно гастропарез [<xref ref-type="bibr" rid="cit56">56</xref>]. Однако у небольшой части пациентов (10–17%) регистрируются лимбический энцефалит и/или рефрактерные эпилептические приступы. Приступы в дебюте заболевания могут быть височными или вневисочными [<xref ref-type="bibr" rid="cit57">57</xref>].</p><p>Anti-Ri (ANNA-2) IgG (нейрональные ядерные антитела типа 2)</p><p>У пациентов с IgG к ANNA-2, или anti-Ri, также были зарегистрированы эпилептические приступы и лимбический энцефалит. Однако типичным клиническим проявлением является синдром поражения ствола головного мозга или мозжечка, включающий атаксию, нистагм, опсоклонус-миоклонус, дистонию открывания челюстей и ларингоспазмы [<xref ref-type="bibr" rid="cit57">57</xref>].</p><p>Ma-2 IgG (антитела к мембранным антигенам)</p><p>Пациенты с IgG к Ма-2 обычно имеют клинику лимбического энцефалита или стволового энцефалита. Авторы ретроспективного исследования Ма-2-IgGсеропозитивных пациентов сообщили о БТКП или фокальных приступах с нарушением сознания у 44% больных [<xref ref-type="bibr" rid="cit58">58</xref>]. Большинство пациентов с IgG к Ма-2 имеют либо опухоль половых клеток яичек (особенно молодые мужчины), либо мелкоклеточный рак легких (пожилые пациенты с сосуществующим IgG к Ма-1) [<xref ref-type="bibr" rid="cit59">59</xref>]. Благоприятные клинические исходы были связаны с отсутствием сосуществующих антител к Ма-1, мужским полом, более молодым возрастом (младше 45 лет) и наличием опухоли яичка с полным ответом на лечение.</p><p>CRMP-5 IgG (антитела к белку – медиатору коллапсинового ответа 5)</p><p>IgG к CRMP-5 (англ. сollapsin response mediator protein) является паранеопластическим биомаркером мелкоклеточного рака легких или тимомы [<xref ref-type="bibr" rid="cit63">63</xref>]. Пациенты с IgG к CRMP-5 имеют различные неврологические проявления, включая лимбический энцефалит, хорею, невропатию черепно-мозговых нервов, мозжечковую атаксию, миелопатию и полирадикулоневропатию [<xref ref-type="bibr" rid="cit14">14</xref>][<xref ref-type="bibr" rid="cit60">60</xref>][<xref ref-type="bibr" rid="cit64">64</xref>]. Также у CRMP-5-серопозитивных пациентов были зарегистрированы фокальные приступы с сохранным и нарушенным сознанием. Решающее значение для лучшего клинического исхода имеют лечение лежащей в основе злокачественной опухоли и раннее начало иммунотерапии [<xref ref-type="bibr" rid="cit60">60</xref>].</p><p>IgLON5 (антитела к белкам IgLON5)</p><p>Болезнь анти-IgLON5 – это редкое и малоизученное неврологическое расстройство, впервые описанное и охарактеризованное в 2014 г. L. Sabater et al. [<xref ref-type="bibr" rid="cit82">82</xref>]. Определяющие заболевание антитела нацелены на белок адгезии нейрональных клеток, функция которого до сих пор не вполне понятна. У пациентов с болезнью анти-IgLON5 выявляются гистопатологические признаки нейродегенеративного расстройства, включая фосфорилирование тау-белка и отложение его в нейронах [<xref ref-type="bibr" rid="cit83">83</xref>]. До сих пор велись споры о том, является ли антиIgLON5 аутоиммунным расстройством, вызывающим вторичные нейродегенеративные события, или главным образом нейродегенеративным заболеванием с сопутствующими специфическими антителами. В недавнем исследовании было выявлено, что IgG к LON5 нарушают цитоскелетную организацию в культивируемых нейронах гиппокампа крыс, что приводит к дистрофическим нейритам и аксональному набуханию. Эти данные наряду с повышением уровня нейрофиламентов в спинномозговой жидкости пациентов свидетельствуют о том, что IgG к LON5, в отличие от других антител к клеточной поверхности, нарушает поддержание цитоскелета нейронов, обеспечивая связь между аутоиммунитетом и нейродегенерацией [<xref ref-type="bibr" rid="cit84">84</xref>].</p><p>Заболевание в основном характеризуется тяжелыми нарушениями сна (парасомния, бессонница, чрезмерная дневная сонливость и нарушение дыхания во сне), бульбарными симптомами (дисфагия и дизартрия) и нарушением походки [<xref ref-type="bibr" rid="cit83">83</xref>].</p><p>Продемонстрирована сильная ассоциация с носительством гаплотипов HLA-DRB1*10:01 и HLADQB1* 05:01, в то время как проявление симптомов и тяжести заболевания носит гетерогенный характер. К моменту постановки диагноза у пациентов выявляются четыре ведущих синдрома: расстройство сна с парасомнией и затрудненным дыханием во сне; бульбарный синдром, включающий дисфагию, сиалорею, стридор или острую дыхательную недостаточность; синдром, напоминающий прогрессирующий супрануклеарный паралич; когнитивные нарушения с хореей или без нее. У всех больных со временем развилась парасомния, апноэ во сне, бессонница или чрезмерная дневная сонливость. HLA-DRB1*10:01 и HLADQB1* 05:01 были положительными у 87% пациентов, аллель DRB1*10:01 был в 36 раз более распространен, чем в общей популяции [<xref ref-type="bibr" rid="cit83">83</xref>].</p><p>К другим иммуноопосредованным эпилепсиям относят энцефалит Расмуссена, впервые возникший рефрактерный эпилептический статус (англ. new-onset refractory status epilepticus, NORSE) [<xref ref-type="bibr" rid="cit104">104</xref>][<xref ref-type="bibr" rid="cit105">105</xref>].</p></sec><sec><title>ЗАКЛЮЧЕНИЕ / CONCLUSION</title><p>Пациенты с аутоиммунной эпилепсией должны регулярно наблюдаться, предпочтительно у эпилептолога совместно с нейроиммунологом. При длительном наблюдении у некоторых больных сохраняется хроническая фармакорезистентная эпилепсия, несмотря на агрессивную иммунотерапию и прием противоэпилептических препаратов. Определить, является ли эпилепсия вторичной по отношению к необратимому повреждению коры головного мозга или вызвана персистирующим воспалительным процессом, может быть трудно.</p><p>При более глубоком понимании механизмов антителоопосредованных и аутоантигенспецифических Т-клеточно-опосредованных синдромов аутоиммунной эпилепсии использование противоэпилептических препаратов и иммунотерапии, вероятно, будет дополнительно оптимизировано. В будущем мы ожидаем, что хорошо разработанные рандомизированные контролируемые исследования помогут в научно обоснованном выборе наиболее эффективных методов лечения аутоиммунной эпилепсии.</p><p>1.
